Official Entry Form

Before completing this entry form, please read and follow the Call for Entries criteria.

The firm, project, client (if applicable) information should be entered exactly as they should appear on official award documents, publications, certificates and trophies. An entry fee of $250.00 per submission is required. All applications and entry fees must be received no later than September 20, 2013 to be considered for an award.

Note: A client/owner letter is required for all entries submitted in Categories A-F and should be included in the bound submission due September 20, 2013.

Please complete the information below. Once the application is submitted, the applicant will be taken to a payment page where there will be an option to pay by credit card or check. Check payments should be included in the submission materials and mailed to:

After MAPPS receives the entry submission, a PDF version will be emailed to the contact name listed on the application to use in the bound submission.

Please accurately complete the required fields. Information provided may be used for publicity purposes and for engravings, should the entry receive an award.

Categories A-F: Final deliverables must have been accepted by the client between January 1, 2012 and May 31, 2013.

Categories G-H: Must have been introduced into the marketplace between January 1, 2012 and May 31, 2013.

Information about your firm will be populated automatically as long as you are signed in to the MAPPS website. Please check for accuracy.

I am aware that a client/owner letter must be submitted as part of the bound submission for Categories A-F, due September 20, 2013. The letter must include the permission to publicize project's details with respect to its complexity, innovative application of technology or techniques, and value to the geospatial profession and the public.

The awards will be announced at the Excellence Awards Reception on November 13, 2013 in Alexandria, VA. I understand that a representative from my firm should make every effort to attend.
